Then you can use this form to write down whatever items you want.
2) When you are ready to order, you can then MAIL the completed
form, or FAX the completed form, or CALL US, TOLL FREE, and read from
the completed form.
3) You can also copy and paste the form into an EMAIL, but DO NOT
INCLUDE your charge card information in the E-mail orders. After
you send the email order, you can then call us to let us know the charge
card information, or we can call you. Just let us know what time of the
day you are available, and be sure you provide us with your phone
